In the event of a death
What to do in the event of an APF member’s death, and what the nominated beneficiary needs to provide.
We’re so sorry for your loss. Here’s what happens next, and how we’ll help.
Step 1
Let us know
Phone the APF Secretary on 1300 013 122 or email secretary@ambprov.org.au.
Email us a copy of the member’s death certificate. If a certificate isn’t available yet, a doctor’s letter, funeral notice or NSW Ambulance Vale Notice is fine.
Step 2
Who can claim
The benefit is paid to the beneficiary named in the member’s APF membership records, directly and not through the estate. If you’re not sure whether you’re the named beneficiary, call us and we’ll check.
Step 3
What we will ask you for
If you don’t know some of these details, that’s fine. Give us what you have and we’ll find the rest.
First name, surname, APF membership number and date of birth
Date of death, and the death certificate or other evidence
Your name, address, mobile number and email
Account name, financial institution, BSB and account number
This is the account the benefit is paid into.
Step 4
Payment
We pay the amount set in the Beneficiary Payment Table at the time of the member’s death. The final amount is approved by the APF Board.
First we pay $10.00 into your account. Once the details are confirmed, we pay the remainder straight away. This makes sure the money reaches the right account.
Ordinarily, we pay the nominated beneficiary within 48 hours of being told of the member’s death. If some details are still to come, it may take a little longer, and we’ll keep you informed.
Need help?
Call the APF Secretary, or email. We’ll talk you through each step.
